New Student Application/Enrollment Information
New students can apply starting Monday, February 3rd.
Due to limited resources we only have a part time academic support team. Students who have an IEP/504B, please check in with our office staff first at 937-252-5400 before starting your online application.
To complete the application process, you will be asked to provide the following information:
- Up-to-date Custody Verification Papers Included (if applicable)
- Completed Adoption Papers Included (if applicable)
- Copy of ETR, IEP or 504 (if applicable)
- Copy of Official Stamped Birth Certificate (all 4 corners of the certificate must be visible)
- Information to make online payment for processing fee

After you have uploaded all documents and completed the application packet, a Submit Packet and Make Payment form will appear. Please follow the instructions that will be provided to submit the application packet along with the non-refundable processing fee as follows:
Once you are contacted by the school that your application has been accepted, you will need the following information to complete the enrollment packet:
- Updated Immunization Records
- Authorization to Disclose Immunization Information and or Revocation Form
- Medical Forms A&B
- Physical Examination Form, required for new students
- EdChoice Scholarship Request Form
- Student is new to the EdChoice Program- EdChoice Scholarship Program Request Form 2025-2026
- If participating in the EdChoice Scholarship Program, Proof of Address is required.
- Submit most recent report card.
- Homeschooled students (not enrolled in an online school)-submit copy of your school district’s excusal letter.
