If you do not plan to return, please fill out the Decline Enrollment Form Here and return to the school.
Decline Enrollment forms need to be submitted by April 1, 2025. Decline Enrollment forms turned in after April 1, will incur a $250 withdrawal fee per student.

PLEASE NOTE BEFORE ENROLLING: Due to limited resources we only have a part time academic support team.
Enrollment will begin on January 27th for all current students.
Log into your Facts Mgt Family Portal and choose the Enrollment tab to begin the process.
New/Returning Student Enrollment Information:
To complete the enrollment process, you will be asked to provide the following information:
- Updated Immunization Records
- Authorization to Disclose Immunization Information and or Revocation Form
- Medical Forms A&B
- Physical Examination Form, required for new students
- Updated Court/Custody Documents, if applicable
- Information needed to make an online payment for the processing fee
- EdChoice Scholarship Request/Renewal Form
- Student is new to the EdChoice Program- EdChoice Scholarship Program Request Form 2025-2026
- Student is a returning EdChoice Student- EdChoice Scholarship Program Renewal Form 2025-2026
- If participating in the EdChoice Scholarship Program, Proof of Address is required. Please be sure to contact Mrs. Hawley if you have had a change of address since applying for the current school year. This may affect which scholarship you are eligible for.
After you have uploaded all documents and completed the enrollment packet, a Submit Packet and Make Payment form will appear. Please follow the instructions that will be provided to submit the enrollment packet along with the non-refundable processing fee as follows:
